Online aangifte hospitalisatie
No, it is not possible to file an online tax return for a deceased family member. In this case, you provide the hospital invoice and the completed and signed declaration form to CM Insurance. This can be done [email protected] or via a CM mailbox.
From your CM profile you can only file an online tax return for yourself and any dependents under the age of 18. Every person over the age of 18 must create a CM profile themselves and file a tax return in their own name.
Are the costs of your newborn included on your hospital bill? Then you can submit that invoice in your name. Was a hospital invoice drawn up in the name of your newborn? Then file the declaration in your child's name.
If your declaration was successfully forwarded, you will see the text on the last screen: 'An email with an attachment has been sent to...'
You will also receive a confirmation to the email address known to CM. Be sure to check your spam folder.
When processing your hospital invoice, it is checked whether the hospital was allowed to charge the medical costs. Do you see the statement 'Member Defense Dispute' on the payment slip from CM Insurance? The Member Defense department will then investigate this cost. Don't you see this? Then your hospital bill is in order.
Are they different hospitalizations for the same condition? Then you can add all of these to one tax return.
Are they different hospitalizations for different conditions? Then you go through the online declaration for each condition.
You can also submit that invoice online. Is the end date of your admission known? Then you can already enter this under 'end date of hospital admission'. Is the end date not yet known and is the hospital admission still ongoing? Then enter the date of declaration under 'end date of hospital admission'.
For your tax return, add the P and G invoices. Have you only received one of the two invoices? Then wait for the second invoice to submit both together. Would you like to declare your first invoice earlier? Then attach a scan so that we have the correct invoice amount.
No, you will receive one confirmation email with several attachments. A summary is included in the appendix for each hospital invoice submitted.
Then you add your invoice yourself as a PDF, JPG or PNG file. Please note , include all pages of the hospital invoice: both 'summary of costs' and the 'detail invoice'.
Some hospitals split the medical costs of a hospitalization over two separate invoices: a P-invoice and a G-invoice. The letter P or G will then appear in the invoice number. A P-invoice contains the costs of accommodation, pharmacy and various other costs. A G-invoice contains the costs of doctors' and paramedics' fees.
Other hospitals work with one hospital invoice for all medical costs.
An accident is an unexpected, unwanted event. CM Insurance defines an accident as a traffic accident, sports accident, industrial accident, accident at school or in the youth movement, private accident (unexpected, unwanted event in the private sphere, e.g. a fall down the stairs).
You can find the invoice number on the 'Summary of costs' page of your hospital invoice. You will find more information about your hospital invoice on Hospital Invoice .
Have you received your hospital bill? Take them by the hand and start filing the tax return. You can read how to do that here .
Based on the 'reason for hospital admission', CM Insurance can correctly calculate the reimbursement of your hospitalization.
The more clearly and concretely you describe the reason for your hospitalization, the better CM Insurance can calculate the reimbursement of the costs of pre- and post-care.
Unless otherwise stated, CM-Hospitaalplan and CM-Hospitaalplan Plus will reimburse the majority of your medical expenses that are directly related to your hospital. You do not need to take any action for this yourself. For CM Hospital Plan, this concerns the medical expenses in the period from 1 month before to 3 months after your admission. For admissions from 1 January 2025, this period will be extended to 2 months before to 4 months after your admission.
CM Insurance automatically processes all these related costs at the following times:
- 4 months after your discharge from the hospital
- 7 months after your discharge from hospital
- 10 months after your discharge from hospital (for admissions from 1 January 2025)
Even better insured? CM-Hospitaalplan Plus provides a refund for the costs of examinations directly related to your hospital admission 2 months before up to 6 months after your admission.
Since CM Insurance receives your online declaration immediately, the processing time for an online declaration is shorter than that of a paper declaration.
CM Insurance never pays the invoice directly to the hospital, you do that yourself. Make sure that payment to the hospital is made on time.
The reimbursement for CM insurance will be transferred to your account.
