We send you your tax form

Did you receive sickness or disability benefits in 2023? Then CM will soon send you a tax form (281.12).

Sending is spread over several weeks. If you chose to receive your CM correspondence by email or Doccle, you will receive the tax form online. If this is not the case, you will receive the tax form by post.

You must have received the form by the end of April 2024. You enter the amounts stated on this certificate in the respective sections of your tax statement.

If you were recognized as 66 percent disabled by the Riziv on January 1, 2024, you must also check the code 1028-39 or 2028-09 (severe disability) to possibly receive tax reductions. For more information, please contact CM
