Report accident
You must report any accident with injuries or injuries to CM.
Medical costs resulting from an accident should not only be reported to the insurance company involved. You must also report any accident with injuries or injuries to CM.
In this way, CM can reclaim the amounts it paid you from the person (or his insurer) who is held responsible for the accident.
An accident can be reported in two ways :
- either provide CM with the completed ' Report of accident ' form;
- or you do that digitally.
Digital accident report
- Download the fillable tax return form , save it and open it with Acrobat Reader.
- Fill in the colored fields.
- Click on the signature field, choose the certificate (name of the person) you want to sign with and click 'Sign'.
- Place your eID in the card reader, type your PIN code and click 'OK'.
- Save the completed and signed PDF.
- Complete the form below, add the declaration and click 'Send'.

You can also receive the form without having reported an accident. This happens if CM suspects that you have had an accident (e.g. based on a hospital admission or a declaration of disability).
Always return the completed form to CM as quickly as possible, even if your medical costs are not the result of an accident.
